Eligibility
• Grade 12 students who are expected to graduate
at the end of the current Academic Year.
• Students who graduated from Grade 12 in the previous
Academic Year or earlier and have not graduated from
any college course from any higher educational institution.
• Passer of the Philippine Educational Placement Test (PEPT),
Alternative Learning System (ALS) or Non-Formal Education
Accreditation and Equivalency (NFEA&E) Program following
DepEd regulations and therefore certified eligible for admission to college/tertiary level.
• Must have a GWA of at least 85% or equivalents.
ADMISSION REQUIREMENTS
• Three (3) copies of latest 2×2 ID pictures
(taken within the last 6 months)
• SHS Form 138
• Parents’ Income Tax Return or Certificate of Indigency
• Certificate of Good Moral Character/ Grade 11 or 12 High School Principal’s Recommendation
• PSA issued birth certificate
• PhilHealth Identification card or membership number
